Letter from a recognized/reputed Indian hospital on the Hospital letterhead with the below details
Letter should be addressed to CONSULATE GENERAL OF INDIA DUBAI UAE.
Brief profile of Hospital.
Patients name as per passport, nationality and Passport no. of applicant.
Disease or Treatment name, Doctors name, center giving the date of consultation and admission in the hospital.
Total expense of the treatment (approximate cost) needs to be mentioned.
Letter must be original only for Bangladeshi National and Pakistan National.
Attend name, nationality and passport number (2 Adult attend only).
Valid medical documents from a recognized UAE hospital on the Hospital letterhead with the below details. (For Medical visa and Medical attendant visa letter of recommendation from local doctor is mandatory)
Brief profile of the Hospital.

Disease or treatment with Doctors name required.
Attend name, nationality and passport number (2 Adult attend only).
Treatment name and hospital specialization should be similar. (Ex-patient coming for cancer treatment and the hospital is specialized in gastric- not acceptable)
Name of the hospital and stamp should be the same.
Bank statement required and the balance in the bank statement should be higher than the treatment cost. If the patient doesn’t have a bank account, they can provide any of their relative's OR a sponsor’s bank statements with NOC letter stating that they would be bearing the total expense of the treatment and travel. In case of housewives they can use their husband's bank statement with a NOC (No Objection Certificate) from husband's side.
Hospitals need to send an invitation letter to Indian consulate with proper stamp and authorized signature keeping applicant in CC. (Print of which has to be attached along with the documents).
For Minors (below 18 years) a NOC (No Objection Certificate) is required with proper content and it has to be signed by both parents. Parent's signature should be the same as is in their respective passports.
Medical Attendant Visa
Document establishing relationship with the patient.
Copy of Medical credentials (In case medical professionals accompanying the patient).
Medical letter from recognized hospital with attendants name included is required.
Note:
Medical attendant visa would be granted to only those people who are accompanying the patient. (Maximum of two medical attendants)
Indian hospital letter should be original or colored on hospital letter head with stamp and signature which is current dated (within 30-days), should specify type of treatment and duration of treatment, passport number, nationality, name of the patient, approximate cost for the treatment and attendant.
